Job Summary/Description

Makes visits to community organizations and provides educational events throughout the 21 counties of West Texas. Works to inform women about the opportunities for breast and cervical screening through community outreach and education. The Community Health Worker serves as program spokesperson and representative to participating health care providers, local communities, and local health agencies.
This position is funded by a grant; continued employment beyond the term of the grant is not guaranteed. Travel is required with some overnight.

Typical Duties/Job Duties

1. Provides resources and referral information and acts as a liaison between the client and the referral agency, the clients’ families and community.
2. Assist clients with patient navigation services of determining eligibility for screening resources, arranging for referrals, scheduling appointments, and assisting with other services.
3. Schedule events throughout West Texas and travel to participate in community outreach in order to promote Access to Breast and Cervical Care program.
4. Inform professionals in the community about the Access to Breast and Cervical Care program and find ways to target the population at need.
5. Keep up to date client files and ensure that all information is kept in accordance with HIPAA guidelines.
6. Performs other duties as assigned.

Knowledge, Skills and Abilities

Knowledge of available West Texas breast & cervical cancer resources. Skilled in oral and written communication. Ability to organize work effectively, prioritizes objectives, evaluate and report activities. Ability to establish and maintain effective work relationships. Interest in, and ability to, educate women about breast health, including screening guidelines, how to perform a breast self-exam and how to access resources. Willingness to present public presentations about the programs and services provided by ABCC4WT.

Minimum Qualifications

A minimum of a High School Diploma.
Excellent communication skills, oral and written.
Ability to counsel clients/callers and answer questions of a sensitive nature.
Must have transportation and be willing to drive to areas within assigned counties (mileage reimbursed).
